Moreover I have ordered the Doctor to also contact the bank and inform them that you will soon be contacting them with an application letter for the transfer of the fund and as well a copy of the deposit certificate as proof and an authorization certificate from me.
Therefore fill below the Application Letter and send to the bank immediately with the deposit certificate. I will be having my last operational surgery next week and I don't know what will come or meet up, either i will come out successful or ???.
